Just like back during the Cold War, the Russians had "showpiece" homes, hospitals, workplaces, that they "showed" to visitors, and made them think that was the way everyone lived. What Moore saw was probably Castro's personal hospital and staff, with a few peasants brought in for effect.
Cuba's own little "Potemkin villages."

I made my "Potemkin Village" comment after looking at the pictures, but before reading the whole post; I just scanned it. Sorry... I didn't mean to imply that others needed more explanation.
